As you read the questions below, please answer Yes/No. We offer three different sliding scale options based on the total of how many questions you answer with "Yes". During the consultation call, you do not need to tell us which questions you answered "yes" to - we will just ask you for the total number which would determine the sliding scale rate.
Are you concerned about being able to pay your bills every month?
If you had no income for a month would you be worried about your upcoming rent/mortgage payment?
Do you have a hard time paying for basic necessities such as food, housing, utilities and medical care?
Do you depend on anyone else for income? (Parents, Adult Child, Government Benefits)
In the last 12 months, was there a time when you needed to see a doctor but could not because of cost?
Do you have to dip into savings to pay for a medical visits, tests, or medications?
Would a major unexpected expense impact your ability to pay your monthly bills?
Would giving a gift for a wedding, birthday or other occasion would put a strain on your finances for the month?
Do you have student loan debt or outstanding medical bills?
Do you qualify for food stamps or social security?
